    Qualifications

    Education

    Graduate of an ACPE-accredited College of Pharmacy with a minimum of Doctor of Pharmacy degree or equivalent experience. Required

    Licenses and Certifications

    Licensed to practice pharmacy in the state of Arizona. Required

    Responsibilities

    Job Summary

    The Ambulatory Pharmacist provides high quality, direct patient care through active participation on the healthcare team to support medication management for patients transitioning between the inpatient and outpatient settings. The Ambulatory Pharmacist will improve medication related safety and reduce hospital readmissions through identification and resolution of medication related problems in the post-acute care setting. Provides medication information to patients and healthcare professionals. Adheres to local, state, and federal laws regarding the profession of pharmacy practice.

    • Identifies medication related problems through comprehensive review of the electronic medical record, medication regimen, labs, patient interview, and other available resources as needed. Medication related problems may involve drug-drug interactions, contraindications, dosing errors, medication adherence, educational needs, allergies, omissions, therapeutic duplications, financial factors, and other issues. Develops a prioritized problem list related to medications and identifies potential contributing factors.
    • Updates the medical record to ensure that an accurate medication list is maintained, contributing to the medication reconciliation process. Reviews current medication list and medication use history, including prescription and nonprescription medications, herbal products, and other dietary supplements. Information is obtained from multiple sources, including a patient’s personal medication list, patient self-reports, pharmacy dispensing system records, facility records, healthcare providers, health information exchanges, and additional sources as appropriate.
    • Provides therapeutic consultation to assist with resolution of medication related problems and improve quality of care in a cost-effective manner. Applies principles of evidence-based medicine to establish patient-specific therapeutic drug regimens. Adheres to the clinical practices approved by the Pharmacy and Therapeutics Committee (e.g. collaborative practice agreement for Transitional Care Management). Collaborates with the healthcare team to optimize medication management. Appropriately documents all patient care activities in the electronic medical record. Demonstrates accurate and thorough documentation skills. Facilitates pharmacist-to-provider communication of clinical interventions and recommendations.
    • Competes patient outreach in a timely manner and proactively plans subsequent follow-up when needed. Conducts direct patient care interactions via telehealth or face-to-face encounters. Provides essential medication education to patients and caregivers to promote medication adherence and safety. Provides educational information to physicians, nurses, and allied healthcare professionals as needed or assigned.
    • Applies the principles of continuous quality improvement consistent with job expectations. Assists in data collection and testing for quality improvement as directed. Completes intervention documentation according to department guidelines.
    • Oversees and directs pharmacy support personnel, including daily activities of technicians or medical office assistants when needed. May serve as a preceptor for pharmacy students and/or pharmacy residents if criteria are met as established by the College of Pharmacy and/or American Society of Health-System Pharmacists (ASHP). Actively participates in the education and training of new pharmacy employees.
    • Performs other duties as assigned.
