    Education
    Master's Degree advanced practice degree, or equivalent in a healthcare field or related discipline Required

    Experience
    3 years of healthcare management experience in patient safety, risk management or a related field. Required

    Responsibilities

    Job Summary
    Responsible for oversight and strategic direction of a comprehensive patient safety program across the network. Ensures a uniform, standardized, and effective approach to adverse event investigation and analysis, including direct supervision of the campus Patient Safety resources & Drug Diversion Specialists. Ensures alignment of patient safety activities and provides opportunities for all staff to be educated and participate in patient safety and quality initiatives. Proactively addresses patient safety issues using data, information from the Patient Safety Committee, rounding and reports, to analyze issues and determine their root causes. Collaborates to develop programs and services that produce measureable improvements in processes and practices that affect patient safety, ultimately resulting in a decrease in numbers, costs and severity levels of healthcare-associated harm incidents and close calls. Collaborates with Risk Management and Human Resources to oversee the Candor, Just Culture, and Care for the Caregiver programs.
    • Oversees the comprehensive patient safety improvement program. Provides leadership in developing a High Reliability Organization, Just Culture and a culture of safety. Provides direction to ensure that clinical services are provided in accordance with safety standards established by federal and state regulatory agencies and National Patient Safety Goals. Key leader for Candor/Disclosure program. Researches, monitors, and assesses applicability of new patient safety methods and techniques. Provides leadership in adaptation and adoption of new methods that enable the organization to accelerate improvement
    • Develops and implements a comprehensive and standardized approach to adverse event investigation including aggregate analysis and trending of data from all sources. Develops and implements both proactive and corrective patient safety initiatives in response to root cause analysis, failure mode and effects analysis, event alerts/recalls DNV survey results, Culture of Safety surveys, AHRQ Patient Safety Indicators, National Quality Forum/Leapfrog Safe Practices, patient grievances, frontline concerns and the event reporting system. Ensures continuous, systematic integration of information about risks and hazards across the system through analysis and trending of data.
    • Co-Chairs and coordinates with the Patient Safety Officer the Network Patient Safety Committee including agenda planning and prioritization of initiatives. Ensures effective communication vertically and horizontally to all constituents within HonorHealth. In collaboration with analytics, develops and maintains a patient safety dashboard as a mechanism for tracking and reporting adherence to patient safety goals. Ensures effective measurement and reporting of safety culture. Provides regular reports to the Quality Committee of the Board and BEST Care Council.

    • Manages external relationships with National and State Patient Safety Organizations (PSO). Provides operational direction for the Patient Safety program. Prepares and administers operating budgets incorporating long and short-term goals and forecasting of personnel, equipment and professional development needs. Manages and monitors the departments’ expenditures to assure fiscal responsibility and accountability. Develops, reviews and implements written policies and procedures for Patient Safety. Key executive responsible for ensuring timely and accurate reporting to the Patient Safety Organization (PSO).
    • Provides essential patient safety and High Reliability Organization communication and establishes partnerships. Meets with leaders as needed/requested to keep them informed of patient safety priorities, initiatives and events. Educates leaders and the Board in patient safety and High Reliability Organization concepts and methods. Supports HonorHealth Committees that address patient safety, quality and risk. Works collaboratively with leaders in Quality, Outcomes/Process Improvement, Infection Prevention, Risk Management, Nursing Practice, Pharmacy and Organizational Development to develop and implement strategies to eliminate harm to patients. Partners with communication and marketing to develop and maintain an internal patient safety website and newsletter and develop patient safety materials and logos to promote patient safety and key initiatives.
    • Fosters and maintains collaborative relationships with external agencies, purchasers, and stakeholders related to patient safety. Key executive responsible for ensuring compliance with Leapfrog requirements and ensuring a successful Leapfrog survey each year.
    • Establishes and maintains a customer service-oriented philosophy for the Patient Safety department in their dealings with all customers.
