    Qualifications

    Education
    High School Diploma or GED Required

    Experience
    6 months 6-12 months health or other human service experience and/or experience with geriatrics population or related experience in an adult health care facility. Required

    Licenses and Certifications
    Other 1) Current certified nurse assistant (CNA) card or current caregiver certificate of completion or have completed 50 hours as Supervisory and Personal Care level caregiver

    Upon Hire Required
    Other 2) Current food handlers card for current employees or must obtain within 30 days of hire for new hires
    3) Current fingerprint clearance card for current employees or application must be provided to ADHC and HR within 30 days from date of hire for new hires
    4) Current BCLS (Basic Cardiac Life Support) card required or must be provided to ADHC and HR within 45 days from date of hire for new hires
    5) Current First Aid card required or must be provided to ADHC and HR within 30 days from date of hire for new hires.

    Please note: if any of the above listed cards expire, they must be renewed prior to the expiration date to maintain eligibility for continuing employment with Adult Day Healthcare (ADHC). Required

    Responsibilities

    Job Summary
    The certified Caregiver-ADHC position is responsible for assisting with the daily activities in the Adult Day Health Care program. Greets the frail, elderly, cognitively and physically impaired members and family members each day; provides and assists with meals and snacks, assists with activities, transportation, and maintains an orderly work area. Assists members with personal care services such as ambulating, transferring, toileting, showering, changing, and feeding. Assists RN with the taking and recording of vital signs and other personal care services as needed. Assists with self-administration and directed administration of medication under supervision.
    • Personal Care: Assists with, or provides as instructed by the ADHC registered nurse.
      •Ensures that all member’s basic needs are being met on a consistent basis as determined by the member care plan, observation, and documentation
      •Personal care services and assistance is provided including ambulating, transferring, toileting, showering, changing, and feeding of members as required
      •Takes all steps necessary to ensure safety of members and reports any significant medical information or changes to nurse as evidenced by documentation
      • Offers and provides meals and snacks to members as directed including feeding assistance and monitoring of intake and special dietary requirements
      •Follows policy and procedure for accurate count of actual meals and snacks served for CACFP billing
      •Follows proper procedures and health sanitation standards at all times in the area of food preparation, serving, feeding, and storage of items as evidenced by documentation and observation
      •Participates in data collection for Quality Assurance
      •Participates in monthly staff meetings to remain current on member needs and gain education on population served
    • Activity: participates in delivering a wide variety of social and recreational activities to members of Adult Day
      •Coordinates with other staff to ensure daily activities are delivered according to established schedules and procedures
      •Leads/co-leads activity groups and monitors members in a range of areas including table games, bingo, active games, musical expression, arts and crafts, and other therapeutic recreational activities
      •Leads/co-leads community outings and events under the direction/instructions of the activities coordinator
      •Works with other program staff to ensure effective communication and coordination of scheduled services as evidenced by feedback and observation
      •Works with activity coordinator to ensure that all scheduled activities effectively meet the needs of all members at a variety of functional levels as evidenced by feedback and observation
      •Assists in maintaining a safe, quality program through complete and accurate activity chart documentation
    • Medical: monitoring of vital signs and medication assist
      •Monitors/documents vital signs (weight, pulse, blood pressure, oxygen saturation, temperature) and reports unusual findings per protocol
      •Under the direction of RN, demonstrates successful ability through observation and documentation to provide assistance with self-medication and non-self- medication
      •Provides medication as directed or observed, which may include preparing and giving insulin shots, breathing treatments, changing oxygen tanks, and other related services within scope of a certified caregiver
      •Recognizes changes in participant’s condition and quickly reports to appropriate person as evidenced by documentation
      •Maintains timely and accurate documentation and recordkeeping; documentation must be accurate and legible
      •Completes health monitoring according to licensure and contracts standards as evidenced by documentation
