High School Diploma or GED Required
1 year 0-1 year experience as a medical records tech or medical office experience or related experience Required
Licenses and Certifications
Other Must demonstrate proficiency in Electronic Health Record post hire by attending required training and passing proficiency test Required
The Medical Records Technician-PN is responsible for ensuring the medical record is complete based on organizational guidelines and standards. Assembles all types of medical records and analyzes the records for documentation deficiencies. Assists providers in completing records and updating the database to reflect completed records. Answers the phone and responds to FAX requests. Responds to requests for information requiring patient authorization or requests exempt from authorization.
- 2.Responds to internal/external release of information requests.
•Releases medical record information in accordance with existing laws governing the confidentiality of medical records as demonstrated by documentation.
•Processes Subpoenas and Attorney requests for medical records
•Assesses the completeness of each consent and authorized by an appropriate party as evidenced by documentation.
•Communicates effectively under sensitive and confidential situations to requesting parties as demonstrated by observation.
•Invoices requesting parties in accordance with established policies as evidenced by observation.
•Maintains Release of Information Database.
•Copies charts for reviews and special studies.
•Works with copying vendor for assigned copying and status of medical records and requests.
- 1.Assembles and analyzes medical records for deficiencies.
•Assembles and analyzes medical records based on regulations governing record completion for all types of patients and clinical services as evidenced by audit.
•Utilizes the computer systems to input and update patient demographics; verifies transcription status and retrieves documents as necessary.
•Assists with the provider status and profile maintenance regarding record deficiencies in accordance with established policies and procedures.
•Pulls medical records for appointments and as requested by authorized personnel.
- 3.Maintains database and provides timely and accurate reports and correspondence regarding completion of medical records.
•Assists in investigating issues regarding status of record deficiencies and retrieves documents to ensure records are complete and accurate as evidenced by audit.
•Assists in reporting changes in provider suspension status to all appropriate parties as evidenced by documentation.
•Assists in the preparation and distribution of letters and notices regarding provider status based on departmental time frames as evidenced by documentation.
•Communicates with provider offices to resolve medical record issues in a timely manner as evidenced by observation.
•Assists in the compliance of medical record completion in accordance with established policies/procedures.
•Ensures transcription is received within 24 hrs, signed by provider and filed in the correct medical record.
- 1.Answers telephone requests and responds in accordance with departmental guidelines.
•Responds to requests following the policies governing confidentiality of health information as demonstrated by observation.Provides assistance to other staff members and may provide training as necessary.
•Interfaces with all departments to ensure a smooth patient record flow. Reports any problems that detract from patient care to supervisor.
- 1.Continually maintains Health Insurance Portability and Accountability Act (HIPPA) and other compliance requirements.
•Enters non-clinical information into patient electronic medical record (EMR) system. Ensures all required information is keyed in thoroughly and accurately.
•Views non-clinical information in EMR system.
•Independently formulates information to be entered into EMR.
•Obtains information to be entered from multiple sources such as providers, nurses, etc.
•Logs, tracks, and resolves issues within specific timeframe to ensure accuracy and timeliness of data entry.
•Brings errors and/or issues to the attention of designee.
•Navigates through multiple complex EMR screens and enters data.Performs other duties as assigned.
