    Qualifications

    Education
    Master's Degree (MS, MN or MSN) from a National League of Nursing (NLN) or Commission on Collegiate Nursing Education (CCNE). Required

    Experience
    1 year Acute Care/Primary Care setting. Required

    Licenses and Certifications
    1.Registered Nurse Licensure in good standing from the Arizona State Board of Nursing or Compact State, Upon Hire Required
    2.Nurse Practitioner Certification in good standing from the Arizona State Board of Nursing,
    and
    3.National Nurse Practitioner Certification in good standing from either the: AANP (Amer Acad of Nurse Practitioners), or ANCC (Amer Nurse Credentialing Cntr)

    Note:
    (a)If NP certified in Arizona on or after 7/1/04 then they must have national certification,
    (b)If NP certified in Arizona prior to 7/1/04, then they are exempt from the national certification requirement
    (c)If NP certified by another state prior to 7/1/04, then they are exempt from the current national certification requirement,
    Required
    3.Prescribing & Dispensing Authority in good standing from the Arizona State Board of Nursing (if dispensing medications), Upon Hire Required
    4.Controlled Substance Registration Certificate from the U.S. Dept of Justice Drug Enforcement Admin (if dispensing narcotics), Upon Hire Required
    5. Current BCLS, ATLS and ACLS Upon Hire Required

    Responsibilities

    Job Summary
    The NURSE PRACTITIONER (NP) works collaboratively with members of the healthcare team to provide care appropriate to the age of the patients served on his/her assigned unit. The practice of the NP emphasizes health promotion and maintenance, disease prevention and the diagnosis and management of acute and chronic diseases. The position is associated with a significant level of autonomy; therefore, the individual must be highly motivated and able to work independently. As an advanced practice nurse (APN) demonstrates leadership as a mentor, educator, researcher and provider. The APN adheres to and functions within the scope of practice defined by the Arizona Board of Nursing and HonorHealth Network privileges. HonorHealth Nursing practices theory based nursing that is evidenced based utilizing a Shared Governance model for decision-making. Practice focuses on performance standards that act in accordance with HonorHealth initiatives.
    • Demonstrates clinical competency in family care settings by providing comprehensive history and physicals examinations with review of systems appropriate to primary care patients in accordance with evidence based treatment guidelines in a Patient Centered Medical Home model. Delivers well care as well as a team centered approach to management of chronic disease states to improve the health of the practice’s patients. Monitors patient outcomes relevant to interventions, patients presenting for follow up care either routine or unplanned by managing interventions appropriately. Manages ordering and review of diagnostic test results as appropriate, under the supervision of a physician. Interacts with patients and families to keep them informed and engaged in the decisions and direction of their health care. Provides counseling education and resource information on wellness, prevention of illness and health maintenance. In partnership with the Physician, refers patients as appropriate for specialty care services and educational training as appropriate for their conditions.
    • Uses disease registries to identify gaps and out of range values in care delivery to utilize support services and care managers to improve metrics of healthcare status.
    • Participates in improving perceived and actual access to care for patients through participation in process improvement to deliver same day appointments and extended hours of availability. Works with the process team to improve efficiencies of care delivery in the office setting to utilize "top of license" care by the office team
    • Work with Practice Development and Improvement Manager on the application process and assist practice in achieving benchmarks toward Patient Centered Medical Home status and NCQA certification.
    • Maintains, monitors and ensures accurate and timely documentation of patient encounter records to meet criteria set forth by HonorHealth Medical Group coding guidelines, CMS, HIPAA and other governmental regulations.
    • Participates in clinical integration within HonorHealth including process improvement, working with care managers, and coordinating transition of care to specialists and inpatient services. Provide timely follow up following inpatient discharge for transition of care to the outpatient setting.
    • Acts in a fiscally responsible manner. Strives to deliver value (quality/cost) to the patient by giving the right care by the right individual at the right time.
    • Regularly attend HonorHealth meetings and events.
