    Doctorate M.D. / D.O. Required

    Member of HonorHealth Medical Staff
    Good teaching skills.
    Board certified or eligible in Family Medicine

    Licenses and Certifications
    Physician\PHYS - Physician AZ State Licensure Required
    Fingerprint Clearance Card - State have a current Fingerprint Clearance card or within seven working days after employment shall apply and be eligible for a Fingerprint Clearance Card. Required


    Job Summary
    Provides direct patient care in a federally qualified health center in accordance with evidence based treatment guidelines. Physician will deliver care in a team-centered approach; interact with patients and families to keep them informed and engaged in the decisions and direction of health care; provide counseling education and resource information on wellness, prevention of illness and health maintenance; and refer patients as appropriate for specialty care services and educational training for their conditions.
    • Clinical Duties:
      Provide direct patient care to eligible patient at the NOAH Clinics in accordance with Community Health Center Standards and evidence based treatment guidelines.

      Maintains, monitors and ensures accurate and timely documentation of patient encounter records to meet criteria set forth by HonorHealth , NOAH, HIPAA and supporting granting agencies.

      Interacts with patients and families, school nurses, teachers, social workers and other outside agencies as appropriate. Provides counseling education and resource information on wellness, prevention of illness and health maintenance.

      Supervises student and resident education rotations at NOAH sites when mutually decided upon with clinician and CMO or designee

      Able to prescribe Medication Assisted Treatment (MAT) (if DEA-X waiver on file)

      Provides supervision for advanced practitioners when mutually decided upon with clinician and CMO or designee

      Interacts professionally and effectively with NOAH clinic staff and management

      Acts in a fiscally responsible manner. Works with management on program sustainment protocols. Performs other related duties as assigned or required
