    Associate's Degree or 2 years' work related experience Required

    Job Summary
    Serves as the primary point-of-contact to the organization's Health Partners member physician practices as they work with the organization. Communicate with the practice staff, office managers, and physicians at assigned practices on a recurring basis regarding administrative processes and topics, and HH technologies. Provides training, answers questions, solves problems, provides informational, procedural and administrative updates and resolves issues with technology solutions. Represents organization as resource to physician practices. In addition to phone and email, will make regular visits to practice office locations.

    Leads practice integration activities by serving as an organizational contact as the practice works toward implementing the technologies and procedures. Coordinates and facilitates training activities (technology and administrative) with community practices. Is responsible for organizing and documenting community practice outreach activities (communications and training) to ensure all practices receive appropriate levels of support
    • Provides day-to-day support to physician offices (both remotely and on-site at physician offices) by providing training, answering questions, solving administrative problems, providing informational, procedural and administrative updates and resolving issues with technology solutions. Facilitates technology related problem resolution for practices by managing contact with vendors, IT resources, etc. to help answer practice questions. Performs on-site visits to increase adoption of technology solutions and processes.
    • In partnership with senior management, implements creative user adoption and change management strategies, tactical plans to support the strategies, and executes tactics. Tracks success of strategies and tactics through usage and status reports.
    • Trains providers and practice users on the use of new technology tools, administrative processes and procedures using a variety of methods including on-site and web conference. Reinforces training as needed. Generates paper and electronic surveys and tabulates results independently to determine effectiveness of training and change management strategies.
    • Keeps manager and/or supervisor abreast of all issues, status of outstanding tasks, and project status reports as appropriate (minimum weekly).
    • Maintains logs of practice interactions to ensure all practices receive the appropriate level of support on a recurring basis.
    • Maintains skills currency and keeps abreast of current issues in Health Care. Identifies opportunities for improvement.
