    Associate's Degree in Business Management, Healthcare Management, Information Technology, or related field Required

    Job Summary
    The Quality & Risk Coordinator is a member of the Quality & Risk Team and will work side-by-side with the Quality & Risk Manager to assist with the team's administrative and operational activities. The Quality & Risk Coordinator assists with developing and tracking practice initiatives, reconciling patient records, and arranging meetings, recording minutes, coordinates the various activities and work throughout Innovation Care Partners' (ICP) provider practices & members and with ICP's payers.
    • Develop communications and documentation for practices to complete monthly practice initiatives, track practice initiative submission, completion, and quality. Answer practice’s questions via email, TigerText, and phone call. Create visual and analytic representations of practice initiatives related information to present to team members and leadership.
    • Manage the Enterprise Master Patient Index by reviewing and determining merges of duplicate patient records .
    • Support department in its administrative and operational activities and provide knowledge and skills for process improvements, arrange meetings and record meeting minutes.
    • Support other department’s operational activities.
    • Performs other duties as assigned.
