    Associate's Degree or 2 years' related experience Required

    Experience
    1 year as Care Coordinator lead or supervisor. Required

    Licenses and Certifications
    Current and valid State of Arizona Finger Print Clearance Card
    Current and valid Basic Life Support (BLS) certification Required

    Responsibilities

    Job Summary
    The Supervisor of Care Coordination is an integral member of the care management team, coordinating with the daily operations and assignments for the care coordinators. This position works collaboratively with the management staff for the Care Coordination program, providers, Care Coordinators and other health care professionals/agencies to ensure smooth and compliant operations to the intensive outpatient care model and guardrails.
    • Assists Care Coordinators in identifying readmission risk and barriers to care outpatient including home support, medication management, expectation, on-boarding process of new practice(s), etc. Coordinates with hospital case manager regarding discharge plans. Monitors and engages care coordinator in IOCP model. Ensure effective post-hospitalization telephonic monitoring, and TCM appointment maintenance for enhanced billing. Reviews documentation, case closure appropriateness, and enrollment volumes to programs
    • Facilitates daily operations of assigned care coordinators, to include managing caseload, ensuring post hospital outpatient follow-up appointment compliance and super visit/shared action planning development. Communicates (verbal, written) information regarding inpatient stay and discharge plans to patient’s PCP/office care manager/health coach. Assures effective transition and final hand-off to the patient’s PCP and his/her office based care manager/health coach. Coordinate with organization(s) (employee plan) or Payer Care Management regarding support desired/required.
    • Facilitates and promotes a collaborative process and communication between all health care team members, inclusive patients/clients, families and significant others to ensure the process of integrated care services are targeted, appropriate, and beneficial to the population served in the Comprehensive Care Coordination Program
    • Performs active listening, uses behavioral motivation interviewing (BMIT) and open ended questioning techniques and guided care goal setting for the patient and ensures aligned care coordinators utilizes BMIT.
    • Maintains all regulatory and compliance educational requirements, participates in continuing education, quality improvement activities, developing and maintaining Clinical Scorecard.
    • Performs other duties as assigned.
